Your Vision Insurance Benefit is provided by VSP to Members. VSP has been rated "Highest in Overall Member Satisfaction Among National Vision Plans" by J.D. Power and Associates. Your annual eye exam will be performed by a VSP network doctor close to you. Your vision benefits include the $15 co-pay for a complete eye exam, per year, per family member and savings on:

• Prescription Glasses
• Laser Vision Correction
• Scratch Resistant Glass Lenses
• Anti-reflective Coatings and Progressives
• Prescription Sunglasses

1. Select a doctor.

2. Make an appointment.
   Call the doctor to schedule your appointment and tell them you are a VSP member.
   
   
3. Pay your $15 co-pay at your appointment.
   The doctor's office will walk you through this process.
   
   
4. Select the frame, lenses or other retail eyewear you want.
   All VSP network doctors have a wide selection of eyewear to choose from available at their facility.
   
   
5. Pay the reduced fee for your eyewear.
